You remember that this is the one about the adolescent with the disclosure, and you remember what the answer was, and you learn nothing. Not a paragraph and four options — a case that develops. What you learn at intake changes what the second session means, and a decision you made early is still there when it turns out to matter.

A “best answer” is an opinion until something outside the question makes it best.
This is the complaint every candidate in this profession has, and it is a fair one. Told for years that the exam rewards “thinking like the test,” people study a bank until they can guess its author. That is not a skill, it does not transfer to the next exam, and it is not what the examination is trying to measure. Every judgment item here names the thing that makes its answer correct — and then you can go and read it.
A code standard, a statute, or a protocol
The clause of the code of ethics, the duty-to-warn provision in your state, the mandated-reporting rule, the risk-assessment sequence, the placement criteria. Named in the rationale, every time.
So FIRST stops being a guess
Once you can see that “first” rests on a stated principle — safety before exploration, consent before disclosure, assessment before intervention — the pattern behind hundreds of vignettes becomes learnable rather than memorizable.
And it survives the codes changing
Ethics codes are revised and practice acts are amended. Material keyed to a source can be rechecked against that source; material keyed to an author’s judgment cannot, which is why so much of this shelf quietly describes a rule that moved.
The source, looked up
A simulated reference browser holds the code standards, the federal privacy and confidentiality rules, and your state’s duty-to-warn statute. Finding the governing provision is a skill the exams reward and the work requires.
Why nobody else does this. Sourcing every judgment item means holding the codes, the statutes and the protocols as real material behind the questions, and rechecking them when they move. It is far cheaper to write a vignette, pick an answer, and call it best practice.
